Toronto Diabetes Care Connect, previously known as the Toronto Central LHIN Diabetes Program, is a regional (city-wide) program that is hosted by South Riverdale Community Health Centre (SRCHC) and funded by the Toronto Central Local Health Integration Network.
We are a Toronto Central regional initiative led by South Riverdale Community Health Centre. We offer programs for community members living with long term health concerns, their caregivers, as well as their health care providers.
