Description of the service and role(s)

Our physicians (MDs) make up a team of family doctors who provide primary care to our clients with services delivered in many different settings, stretching from their own offices, clients’ homes, and other facilities in the community. The role that physicians play at SRCHC is fundamental in the promotion of clients’ health and well being as well as the prevention of illnesses by providing a variety of services ranging from the physical, mental and social aspects of their health.

Is virtual care available

Phone and video appointments are available to clients who require or prefer virtual care. In-person appointment may be required or suggested based on the reason for the appointment.

What is unique about our services (if anything)

Our physicians work with an interdisciplinary team of health care and social service providers, which allows the family doctors at SRCHC to more fluidly integrate many needed services into our clients’ care.

Who you serve / are there common populations you serve

While SRCHC’s physicians serve clients of all walks of life, they strive to ensure that the healthcare they provide is particularly accessible to those who have difficulty accessing healthcare elsewhere. These populations include individuals who face food insecurity, are underhoused, have limited financial resources, are uninsured, are part of the LGBTQ+ community, or face other means of marginalization.

How you accept new patients (referrals?)

Individuals needing a physician can connect with our medical secretaries to complete a registration form. We currently have a wait list of clients who are seeking primary care with SRCHC.
