Description of the service and role(s)
Our clinical Registered Nurses contribute to a collaborative group of clinicians on our primary care team. They add a great variety of skills and specialized training which allows for the integration of wound care, a service which the clinical nursing team provides primary care clients with wounds that range from mild to moderately complex.

Wound care is a service which requires in-person assessment and care.
What is unique about our services
Many other primary health practices may not have the resources to attend to their clients’ wounds and end up referring their clients to other facilities such as hospitals and home and community care agencies. With our clinical nursing team having the skill set and resources to provide wound care on-site the need for outbound referrals and external coordination is reduced. Additionally, our clinical nursing team is able to occasionally able to offer home visits for initial wound assessments for homebound clients. The nursing team provides wound care services to primary care clients who are uninsured and encounter barriers accessing wound care elsewhere, and is available to the rest of the primary care team for consultation on best practices in wound care.
Who you serve / are there common populations you serve
Our clinical nurses providing wound care serve individuals who access primary care with one of our physicians or nurse practitioners, and who need regular dressing changes for their wounds.
How you accept new patients (referrals?)
If you are a primary care client of SRCHC and think you may need to access wound care with one of our clinical nurses, please book an appointment with your doctor for an assessment first.
