Diabetes Education

DECNET, the Diabetes Education Community Network of East Toronto, is a diabetes education program that provides counselling, education and support to help adults manage their type 2 diabetes or prediabetes.

Drug Use

Supports for people who use drugs and require primary care, supervised consumption, sterile supplies and naloxone, referrals to addictions and mental health services, counseling, wound care, foot care, Hep C treatment, and outreach clinic access.

Environmental Health

Environmental health promoters inform people about potential health risks in the home, workplace and community.

Food

Meal supports, cooking skills, nutrition, culturally appropriate supports, social skills, community meals, food security and education.

Harm Reduction

Harm reduction is a set of practical strategies and ideas aimed at reducing negative consequences associated with the criminalization of drug use.

Harm Reduction is also a movement for social justice built on a belief in, and respect for, the rights of people who use drugs.

Harmony Community Food Centre

Harmony CFC programs support local families and individuals by increasing access to healthy food, skill-building, knowledge sharing, and providing leadership and advocacy opportunities for those of all ages.

Wellness, counselling, addictions programs, referrals, and social work supports.

Ongoing Health Concerns

Supports for people with chronic health concerns such as diabetes, prediabetes, asthma, chronic obstructive pulmonary disease (COPD), and cancer. Services include chiropody, physiotherapy, diabetes programs (education, diet and nutrition, foot care, eye exams, counselling, prediabetes management, training, peer programming, and referrals).

Pregnancy

Access to midwifery, pregnancy testing, labour and birth care, LGBTQ2S+ parenting/pregnancy, surrogacy, counselling, housing support, crisis intervention, resource navigation related to pregnancy and parenting, pregnancy-options counselling, access to medication abortion, referrals for surgical abortion, and one-on-one post-abortion counselling.

Recreation

Fitness, leisure, social activities, special events, seniors support groups and exercise classes.
