Toronto Community Hep C Program (TCHCP)

The Toronto Community Hep C Program (TCHCP) is an innovative, community-based, client-centered collaborative care model which provides HCV support, education, and treatment to individuals living with, or are otherwise at risk of acquiring Hepatitis C infection.

We work with people who have had difficulty accessing the mainstream health care system. TCHCP clients are often very low income or homeless, and have current or past lived experience with substance use – thereby navigating routine stigma and discrimination by health and social service systems, along with other historical and ongoing structural oppressions that only serve to shame and blame people for their Hep C status. We practice within a harm reduction framework and use a facilitated peer support model.

We accept referrals from individual providers and community agencies as well as from clients themselves.

Drop-In Clinic Hours:

  • Tuesdays at Regent Park CHC (465 Dundas Street East) from 12 – 4 pm
  • Wednesdays at South Riverdale CHC (955 Queen Street East) from 12 – 4 pm
