South Riverdale Community Health Centre (SRCHC) is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Health Access Taylor Massey
As part of the Ministry of Health’s efforts to expand access and attachment to primary care, SRCHC is working with our partners at East Toronto Health Partners Ontario Health Team and the East Toronto Family Practice Network to hire Physicians to work at Health Access Taylor Massey (HATM). The goal of HATM is to enhance and strengthen access to primary and community care by delivering comprehensive, team-based services and leveraging existing community supports. Rooted in a strong foundation of primary care, HATM is designed to be a sustainable and collaborative approach that addresses both medical and social determinants of health. This integrated care model was developed in response to locally identified health and wellness needs and priorities.

The vision of HATM is to advance health equity and improve population health by connecting residents of Taylor Massey, Crescent Town, Oakridge, and surrounding neighbourhoods with coordinated services offered by partner organizations, in their local neighbourhood.

SRCHC is currently looking to recruit salaried physicians to work at Health Access Taylor Massey.

This position will report to SRCHC’s Director of Integrated Primary Care and will be expected to work closely with HATM’s Medical Director who is responsible for supporting program implementation, strategic leadership and overseeing clinical care for clients accessing services.

Position Summary
Physicians working at Health Access Taylor Massey will contribute to clients’ health and well-being through assessment, prevention, education, rehabilitation and or/ therapeutic support services consistent with the standards of care outlined by the college of Physicians and Surgeons of Ontario. These services are part of an interdisciplinary team approach to client care. Services are provided to clients, target community groups and priority populations identified by the HATM Governance Committee.

At Health Access Taylor Massey the team recognizes that health is shaped by a broad range of social and economic factors, including education, employment, food security, and social support. HATM takes a holistic, community health approach, to promote health/well-being, prevent illness, and strengthen community capacity and connection. The HATM integrated care team’s work is grounded in values of equity, inclusion, community engagement, accountability, transparency, excellence, and collaboration.

Responsibilities:
Direct Client Care
• Ensures that standards of client care are maintained according to accepted College of Physicians and Surgeons of Ontario standards and policies of SRCHC and the Regulated Health Professions Act
• Diagnoses and treats acute and/ or chronic conditions, incorporating health teachings as appropriate
• Utilizes a health promotion approach, addressing determinants of health
• Provides evenings, occasional weekends and after-hours medical care for clients in rotation with other MDs
• Makes internal referrals to other HATM staff and community partners as appropriate
• Refers to therapeutic counselling or short-term crisis management when required
• Participates in case conferences with other providers to ensure coordination of services to the client
• Performs virtual consultation, when necessary, which may include assessment and advice
• Performs necessary back-up clinical functions for other clinical staff in their absence
• Incorporates health care advances into practice and bases this practice on the changing demographics and evolving needs of the community

Health Promotion and Education
• Collaborates with HATM service team and other teams across the organization to identify and provide health education to target groups
• Participates in community projects or agency activities as a representative of HATM and the Health Centre as needed
• Assists in the development of community outreach programs based on the needs of individuals and the community
• Participates in the development of, and gives support to policies and strategies that are in the interest of public health and recognize the determinants of health

Organizational Responsibilities
• Maintains complete and accurate records sufficient for timely communication with other professionals
• Ensures timely completion of statistical and electronic health information as required
• Participates in all relevant HATM and SRCHC programs, activities, meetings, professional associations, and workshops as required
• Participates in the development of clinical policies and procedures as required
• Contributes to the team’s use of research methodologies and findings in clinical practice, and to planning, monitoring and evaluation
• Ensures client safety
• Performs clinical management tasks in alignment with Infection Prevention and Control (IPAC) procedures

Other Requirements
• Maintains current and appropriate college membership as regulated under the Regulated Health Professions Act, 1991
• Maintains malpractice insurance with the Canadian Medical Protective Association
• Maintains continuing medical education (CME) credentials as expected by CPSO and all other required memberships in order to practice
• Supports and/or supervises student placements within HATM and SRCHC as required
• All other duties as required

Qualifications:
• Doctor of Medicine degree from a recognized university
• Current registration and in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
• Membership and Certification from the College of Family Physicians of Canada (CFPC)
• Member of Ontario Medical Association (OMA)
• Member of the Canadian Medical Protective Association (CMPA)
• One (1) year experience post-medical license
• Experience in primary health care and proficiency in conducting the duties of a primary health care physician
• Commitment to and knowledge of community-based health care, health promotion, harm reduction, social prescribing and community development
• Ability to deal effectively with people experiencing crisis, mental health challenges and/or diverse cultural interpretations of health
• Proficiency in the use of Electronic Medical Record computer technology and various software applications
• Experience teaching through the Department of Family and Community Medicine, UofT considered an asset
• Strong verbal and written communication skills
• Excellent interpersonal skills
• Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
• Ability to work well with other staff to ensure an effective and efficient working environment
• Ability to work effectively in a multi-disciplinary environment
• Fluency in a language relevant to the HATM and South Riverdale CHC community, in addition to English, considered an asset
• Police clearance documentation will be required for employment
• Proof of double COVID-19 vaccination status will be required for employment
