Health Access Taylor-Massey (HATM) is a neighbourhood care hub in Crescent Town, where South Riverdale Community Health Centre is a proud partner. With all of their partners, HATM is increasing access to family doctors, nurses, mental health care, and community services in one accessible location.

In Crescent Town and the wider Taylor-Massey community, many people are building new lives while navigating real barriers — language, income, housing pressures, and the challenge of finding primary care. For some, that has meant waiting too long to seek help or turning to emergency departments when things become urgent.

In 2025, HATM received primary care expansion funding to attach individuals to primary care. Through this investment, SRCHC was able to hire additional doctors, nurse practitioners, and a health promoter, strengthening local capacity and expanding access to care for the community.

Together, HATM and its partners, are working toward a more sustainable, community-centred model of care. More than a health space, HATM is a place for relational community — bringing care into the heart of the neighbourhood and strengthening connections between people and the services they need most.
