About South Riverdale Community Health Centre

We provide health care and social programs across multiple locations in East Toronto, helping people who are often underserved by systemic barriers in traditional health care systems.

Bringing compassionate care and connection to every corner of our community

Team-based primary care is who we are. Our programs reflect the many ways health can be improved in your life, whether it’s needing medical care, managing a chronic condition, finding somewhere to treat or manage substance use, joining a seniors’ walking group, or getting support during pregnancy. We offer services that blend medical care with other supports, including social connection, harm reduction, mental health support, access to food, and more.

Our team includes doctors, nurse practitioners, midwives, nurses, social workers, chiropodists, physiotherapists, dietitians, health promoters, and other professionals who share a deep commitment to accessible care, supports, and dignity. We provide support to those navigating complex challenges, such as poverty, discrimination, housing instability, and other barriers, and we do our best to meet each person where they are, with care that’s flexible, culturally safe, and available in multiple languages.

Who we serve

People without OHIP or a family doctor

You don’t need health insurance or a regular doctor to receive care. SRCHC is a safe place – a place you can turn to when other options feel out of reach. For more information on access, please visit the pages for the programs and services you’re interested in.

Seniors and people with chronic illness

We offer programs that help you stay active, manage long-term conditions, and stay connected with others.

Newcomers and people who speak languages other than English

Starting fresh in a new country can be overwhelming, especially when it comes to health and community services. We understand the challenges and offer care and supports that are welcoming, respectful, and mindful of what you might be going through.

2SLGBTQ+ community members

Our care is non-judgmental and inclusive, with supports that respect your identity and lived experience.

People who use substances, or people who are unhoused

We meet people where they are, offering safer use, treatment and support without stigma or judgment.

Families with young children or perinatal needs

From pregnancy to early parenting, we offer care, supplies, and guidance for families in our community.

People facing isolation, poverty, or substance use challenges

Whether you need someone to talk to or help with basic needs, we’re here to support your overall well-being.

Anyone in the area who is looking for social connections or needs accessible, community-based care

Whether you’re looking to build social connections, navigating short-term challenges or in need of longer-term support, you’ll find programs, care and a sense of community that meet you where you are.

Why our work matters

We’re here for people who face systemic barriers to care – whether because of poverty, stigma, homelessness, language barriers, or other challenges that make it harder to get care. The way we approach our work improves health outcomes for people who are often overlooked or underserved, and it eases pressure on emergency rooms, hospitals, and other parts of the health system.
