Overview:

Toronto Diabetes Care Connect makes connections to diabetes supports by operating the Toronto Central Referral Service. Diabetes programs and supports are located in multiple locations across Toronto. Our program helps connect people with programs near them.

We also run professional development programs for healthcare providers and support projects and initiatives that improve access to and delivery of diabetes care at the systems level. For more information about programs and initiatives, please see our annual report.


How to get connected to diabetes support:

We accept self-referrals from people living with or at risk of diabetes, caregivers referrals, and healthcare provider referrals to diabetes supports across the Toronto Region. Referrals are timely (processed within 2 business days) and people are connected to programs that support prevention and diabetes management.
