Overview:

The Primary CONNECT team includes a physician, nurse practitioner, registered nurse, community health worker, social worker or case manager, service navigator, psychiatrist, and Indigenous Healer. The team works collaboratively across three sites and clients can receive support from any combination of providers based on their needs. Each client is connected with one of two Most Responsible Providers (MRPs) to ensure continuity of care.

Eligibility:

Primary CONNECT is for individuals who are not currently connected to a primary care provider or whose current care is not meeting their needs. It serves people aged 16 and older who are experiencing moderate to severe mental health concerns and/or substance use challenges, including opioid or alcohol use. The program prioritizes those who are marginalized, racialized, low income, or who identify as Indigenous, First Nations, Métis, or Inuit.
