Choose Health Toronto Central is a regional initiative led by South Riverdale Community Health Centre. As one of the self management programs for Ontario Health - Toronto Region, Choose Health offers evidence based programs in person and virtually for community members living with long term/chronic health conditions, their caregivers as well as their health care providers.

Program FAQ

Who leads the chronic disease/pain programs?
The program is led by two facilitators who also live with long term health conditions/ pain or are taking care of someone with those conditions. Facilitators attend 35 hours of training and receive ongoing support.
Do we need to pay for the materials?
The materials are free, and you’re welcome to keep them after the program ends. We encourage you to review the handbook from time to time, as it will help you to reinforce what you’ve learned and stay on track with your goals.
Can I miss a session during a chronic disease/pain series?
We encourage you to attend all six sessions to get the full benefit of the program. However, if you need to miss a session, please inform the facilitator in advance so they can advise you on how to make it up.
