Connecting communities to care: SRCHC’s new mobile health clinic hits the road

For nearly 50 years, South Riverdale Community Health Centre (SRCHC) has been meeting people where they are — breaking down barriers to bring care to those who need it most.

Now we’re taking that commitment even further with the launch of our new Mobile Health Clinic, designed to deliver primary care, health promotion, screening, and vaccines directly to communities across East Toronto.

The Mobile Health Clinic represents an innovative approach to community care, making healthcare more accessible for people living in high-needs neighbourhoods, often with limited transportation options.

“This initiative is  all about removing barriers to care,” says Shannon Wiens, SRCHC CEO. “By going directly into communities, we’re creating new pathways to care for those who might otherwise go without. It’s another way we’re responding to the real needs of our community and ensuring care is accessible to everyone, no matter their circumstances.”

The Mobile Health Clinic hit the road this fall, supporting two community flu vaccine clinics in partnership with local organizations. In October, we worked with Red Door Family Shelter to provide flu vaccines and health information for both residents and staff, offering a safe and welcoming space for care.

Then in November, the clinic joined WoodGreen Community Services at their Cedarvale-Greenwood location to deliver flu vaccines for residents, continuing our shared focus on prevention, access, and community well-being.

“Partnerships like this show what’s possible when we work together,” says Rachel Seewald, a nurse practitioner on SRCHC’s integrated primary care team. “By bringing vaccines and other essential health services directly to people, we’re building trust, strengthening connections, and helping make care more accessible for everyone.”

The Mobile Health Clinic will continue to travel on a regular rotation and support health screenings, education, and other wellness initiatives throughout the year.

As SRCHC approaches its 50th anniversary in 2026, the Mobile Health Clinic reflects both a continuation and an evolution of our mission: to ensure equitable, community-based care that meets people where they are and brings health services closer to home.
