Creating a Future Where Everyone Belongs

For more than 50 years, SRCHC has grown alongside our community, responding to changing needs with compassion, partnership, and a commitment to health equity.

Today, our community is facing new and growing challenges. Housing insecurity, rising living costs, increasing demand for care, and persistent barriers to health continue to affect the well-being of many people. These realities call on us to listen, adapt, and work together in new ways.

Our Strategic Plan for 2026–2031 draws on the values that have guided us for decades while setting a clear direction for the future. Together with our clients, staff, Board of Directors, volunteers, partners, and community, we are strengthening the systems that support people, improving access to care, and creating healthier, more connected communities where everyone belongs.

Healthy communities depend on strong, connected systems of care.

We are helping shape a more connected, responsive system of care across East Toronto. Through deep collaboration with partners, we are weaving together an ecosystem of support — from primary care to mental health, housing supports, and social connection — so people can access the full spectrum of services they need to thrive.

Connecting care to the realities of people’s lives

Care should be welcoming, accessible, and centred around people's lives.

Care should feel seamless, wherever it begins. By linking our sites, expanding outreach, and aligning our programs, we are creating a consistent experience of welcoming, barrier-free care. From mobile services to programs for newcomers, seniors, and people facing complex mental health and substance use challenges, we meet people where they are — and walk alongside them.
