Eligibility:
This program is available to SRCHC clients who live within the catchment area. A referral from a healthcare provider, such as a doctor or nurse practitioner, is required. Please speak to your primary care provider to make an appointment.
Services offered:
We provide screening and assessment for asthma and COPD, including spirometry testing (a common breathing test). Services include education on these conditions, support for managing symptoms and triggers, guidance on respiratory medications, and training on how to use puffers correctly. We also help create personalized treatment plans, offer consultations and spirometry training for primary care providers, and host community workshops, education sessions, and screening events.
