Getting chronic disease support at SRCHC
Whether you’re dealing with chronic pain, trying to manage blood sugar, or just feeling worn out by appointments and medications, it’s easy to feel overwhelmed. At SRCHC, we offer support that looks at the whole picture: your health, your food, your mental well-being, and your daily life. Our staff and peer-led programs are here to help you develop skills, build community, and feel more in control, one step at a time.

- Do I need a doctor to use your services?
You don’t need a referral or a family doctor to access most of our programs. You can refer yourself or be referred by a health care provider . If you’re not sure where to start, feel free to reach out and we’ll point you in the right direction.
- Can I get help if I don’t have Ontario health insurance?
Yes. We offer care and support even if you don’t have OHIP.
- What kinds of chronic conditions do you support?
We support people with many long-term conditions, like diabetes, asthma, and COPD.
- Are there programs for people newly diagnosed?
Yes. Some of our programs are great for people just starting to learn about their condition, including education and one-on-one support.
- Can I join a program if I have more than one health issue?
Absolutely. Many people we work with are managing more than one condition at a time.
- Is it okay if I have trouble getting around?
Yes. Many of our spaces are wheelchair accessible, and many programs are gentle or low-intensity. Let us know what support you need, and we’ll do our best to accommodate you.
- What if my chronic disease is affecting my mood or mental health?
You’re not alone. We’re here to listen and support people living with chronic diseases. It’s okay to talk about how you’re feeling.
- Are the programs just for seniors?
No. Our programs are open to people of all ages living with chronic disease, including younger people.
- Can I bring a family member or support person?
Yes. You’re welcome to bring someone with you if it helps you feel more comfortable or supported.
- How do I find out what programs are happening and when?
You can visit our website to view all program and event descriptions. Each program includes details regarding when and where they will take place and if you need to sign up in advance. If you’re unsure or have any questions, give us a call.
