New clients:
Individuals needing a primary care provider can connect with our medical secretaries to complete a registration form. We currently have a wait list.
Home visits:
Home visits for those that reside in our catchment that have been onboarded to a primary care provider.
Virtual care:
Phone and video appointments are available to clients who require or prefer virtual care. In-person appointments may be required or suggested based on the reason for the appointment.
Outreach Clinic:
Operated by physicians, nurse practitioners, clinical assistants, medical secretaries and a social worker, this drop-in clinic delivers holistic primary care to local residents who meet specific criteria. Visit our Outreach Clinic page.
Urgent care:
Two hours are dedicated daily, Monday-Friday to provide same day/next day access to care for registered clients who need immediate care but don’t have an appointment. Visit our Urgent Care page.
