Hepatitis C

The East Toronto Hepatitis C Program is a partnership between South Riverdale Community Health Centre, Regent Park Community Health Centre and Sherbourne Health Centre. It was established in 2006  to assist people living with Hep C, who primarily use drugs and or alcohol and or have mental health issues/illness and or HIV/HCV Co-infection and who have had difficult accessing treatment and care. It is a multidisciplinary program offering medical care, treatment, support and education all under one roof.  We work in a holistic and harm reduction framework.

The program is centered around group support. People attend a weekly group focused on information about HEP C, treatment and healthy living. TTC and food are offered. The group is a closed group and runs on a 16 week to 18 week cycle. The Hep C Clinic runs at the same time. 

Hep C Patients can have access to:

  • Hep C Treatment Nurse

  • Case Manager
  • Infectious Disease Specialist
  • Psychiatrist
  • Hep C Coordinator
  • Hep C Community Support Workers.

  • Informal counseling
  • Harm reduction counselling and supplies
  • Group support
  • Patient accompaniments
  • Feferrals to doctors, nurses, counselors / case managers, detoxes, housing workers, shelters

Program Eligibility: 
Hepatitis C-positive people who have had difficulty accessing treatment and care for their Hep C. Priority for people who are uninsured, use substances and or have mental health issues.
Program Offered in the Following Languages: 
English
Program Location: 
South Riverdale Community Health Centre 1st floor, 2nd floor for clinic and 4th floor for group support.
Program Time and Hours: 
Group and Hep C clinic Wednesdays, 1 p.m. to 3 p.m. Hep C drop in for Hep C information and referrals Mondays 10am-12pm Staff onsite Mondays, Wednesdays and Thursdays
