You will find contact information for community health and support services for families with young children in East Toronto and for older adults and their family members living in the South Riverdale neighbourhood.

MATCH (Midwifery and Toronto Community Health)

The MATCH (Midwifery and Toronto Community Health) Program provides complete prenatal, labour, birth and postpartum care with a team midwives. Midwives are experts in caring for pregnant people and their newborns. Chester School Clinic

The clinic at Chester ES is part of the Model Schools Paediatric Health Initiative, which aims to improve access to healthcare for students and their families by offering in-school clinics in neighbourhoods across the city.

Sessions on health, wellness, healthy eating, Osteoporosis, management of chronic conditions, wills and the law, polypharmacy, etc.

Choose Health

We are a Toronto Central regional initiative led by South Riverdale Community Health Centre. We offer programs for community members living with long term health concerns, their caregivers, as well as their health care providers.

Peer Nutrition Program

The Chinese Peer Nutrition Program provide free nutrition education for Chinese speaking parents and caregivers of children 6months to 6years of age living in the South Riverdale area.

Grandparents Support Group

We offer a drop-in support program for a multi-cultural group of grandparents caring for grandchildren of ages 0-12 years every last Monday of the month.
