First Step to Home is a transitional housing program for men 55 years and older who have been street involved and are experiencing mental health or addictions issues.

SRCHC programming offered across Toronto includes Choose Health Self-Management Program, DECNET (Diabetes Education Community Network of East Toronto, Diabetes Eye Screening Program, and Diabetes Care Connect Referral Service. These programs are social and community services that focus on wellness, disease prevention, and skills development for health care providers.

Diabetes Eye Screening Program – Teleophthalmology
Yearly diabetes eye screenings can reduce the risk of diabetes-related damage to the eye (retinopathy) which, if left untreated, can cause vision loss.
Self-Management
Choose Health Toronto Central is a regional initiative led by SRCHC. We offer programs for community members living with long-term health concerns, their caregivers, as well as their health care providers.
Toronto Diabetes Care Connect
Toronto Diabetes Care Connect is a regional (city-wide) program that is hosted by SRCHC and funded by the Toronto Central Local Health Integration Network.
