South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Community Health Worker – Choose Health will support program planning for the city-wide service at SRCHC and in collaboration with other CHCs, grassroots organizations, primary care, social, and community services. This position requires providing support to peer facilitators to deliver a range of evidence-based services to marginalized populations and their healthcare providers.  Choose Health is a regional service in Toronto that offers patient education programs and healthcare provider training to enhance patient self-management. This work takes place in partnership with primary care as well as community based health and social service providers in Ontario Health – Toronto region. Primarily serving equity seeking clients who live with long term health conditions, their caregivers, and healthcare providers who live and work in Ontario Health – Toronto region.

 

This work will require working onsite at SRCHC locations during COVID-19.

 

Responsibilities:

  • Active outreach to marginalized community members living with long term health conditions in order to refer to available self-management programs
  • Network with internal and community partners to ensure referral to available self-management patient education programs and healthcare provider trainings
  • Routinely compile and enter data as per program directives and organizational requirements
  • Participate as a team member in all team functions – program planning, team meetings, facilitator meetings, and partner meetings
  • Maintain and develop professional competence and learning
  • Facilitate recruitment of peer facilitators
  • Facilitate ongoing peer facilitator training and coaching sessions
  • Support and mentor a team of facilitators with lived experience
  • Organize and process facilitator invoices
  • Community development
  • Collaborate with teams across partner organizations, e.g. health promotion staff, community development leads, program administrators, etc.
  • Routinely maintain and update selfmanagementtc.ca to promote upcoming chronic disease
  • Support delivery of virtual programs
  • Other duties as required

 

Qualifications:

  • Post-secondary education from a recognized University or College or equivalent experience
  • 3 – 5 years relevant training and experience
  • Proven skills in group facilitation and in capacity building, peer support and training
  • Strong desire and proven ability to work effectively with people who are equity seeking and live in neighbourhood improvements areas of Ontario Health – Toronto region
  • Proficiency in the use of computer technology and various software applications
  • Proficiency in the use of WordPress and webinar such as Zoom and GoToWebinar
  • Strong oral and written communication skills
  • Excellent advocacy and negotiation skills
  • Demonstrated ability to work independently and as a team member within a multi-disciplinary environment
  • Thorough knowledge and understanding of the local health and social service sector, government programs and current legislation that may affect clients
  • Strong commitment to health promotion, community development and adult education
  • Facility in a second language an asset
  • Police clearance documentation will be required for employment
