South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Primary Care Asthma Program (PCAP) is a program delivered in a number of Community Health Centres in the East End of Toronto and within diverse communities and settings.  The PCAP Coordinator works along with primary care providers to assess, diagnose and educate clients and their families or caregivers about asthma, COPD and smoking. This position fosters a collaborative approach to respiratory health and promotes/utilizes opportunities to increase awareness, skill and knowledge within the community.

 

Responsibilities:

  • Provide asthma and COPD education to clients, families and care providers utilizing best practice strategies and standardized PCAP tools, in accordance with PCAP Advisory Group recommendations
  • Conduct spirometry testing in accordance with the Canadian Thoracic Society (CTS/ERS) guidelines
  • Work with program sites to ensure adherence to Primary Care Asthma Project (PCAP) standards and protocols
  • Work with clients/families and primary care providers to develop, implement and revise customized self-management plans (Action Plans)
  • Assess and address potential barriers to asthma management, including the social determinants of health
  • Operate as a resource person for respiratory health education for SRCHC and the local community, including asthma, COPD and smoking cessation
  • Participate in the implementation of a cross-organizational smoking cessation strategy
  • Identify community resources and help clients understand how and when to best access resources appropriately
  • Complete documentation in accordance with PCAP standards and site specific policies and practice
  • Participate in regular data collection/analysis and generate relevant reports as required
  • Develop and maintain collaborative internal and external partnerships
  • Participate in research opportunities when the opportunity arises
  • Attend relevant events, meetings and participate as a member of internal and external committees and/or working groups
  • Ability to plan, organize, lead and evaluate group programs
  • Participate in outreach activities/activities
  • Liaise with management, directors and administrators at SRCHC, East End and Regent Park CHCs to ensure that resources, policies, and practices for respiratory care and management are in place

 

Qualifications:

  • Registered Respiratory Therapist (preferred) and Certified Respiratory Educator (required)
  • Minimum 3 years clinical experience working with clients conducting spirometry, asthma management, treatment, and education
  • Experience with smoking awareness/cessation strategies (TEACH/STOP certification/training preferred)
  • Undergraduate degree in a relevant discipline from a recognized university or a combination of education and relevant experience
  • Licensed Ontario Regulated Healthcare professional
  • Proven ability to work independently and within an inter-professional team
  • Ability to provide effective and accessible education/care to patients, families, community groups, and health professionals based on current best practice recommendations
  • Ability to accurately and safely perform spirometry testing in accordance with the ATS/ERS Standards
  • Understanding of the intersection between a range of health challenges in order to improve the prevention and management of chronic diseases across the continuum
  • Experience working with diverse clients and communities
  • Knowledge of social determinants of health issues
  • Understanding of concepts of health promotion, patient education, research and secondary prevention as a part of primary care
  • Proven administration skills including participation in meetings, maintenance of client care systems, preparation of project reports, evaluation of clinical care delivery relative to guidelines or standards of care, etc.
  • Proficiency in the use of computer technology and various software applications
  • Strong oral and written communication skills
  • Excellent time management skills; flexibility
  • Police clearance documentation will be required for employment
