Diabetes Programs

Information, programs and services to help you manage your diabetes, feel better and avoid complications. All programs and services are free, and open to caregivers and family. Some diabetes services and programs are offered in Chinese, Tamil and Urdu/Hindi. Interpretation is also available in most languages with advanced request. Program locations vary. (Please see programs for specific locations.)

Available Programs:

Diabetes classes led by a Chinese-speaking registered dietitian. Learn how to feel better and avoid unnecessary risks through healthy eating. Families and caregivers are welcome. 7 sessions (2 ½ hours each).
Prepare healthy and delicious meals in our community kitchen with a dietitian and other people living with diabetes. Share recipes and tips and learn more about managing diabetes through healthy eating.
Speak one-to-one with a dietitian about managing diabetes through healthy eating.
Talk to a diabetes nurse educator and learn about diabetes wellness, as well as community resources and services.
A general assessment is an initial, individual appointment with a diabetes educator.
A monthly support group for people with type 2 diabetes. Family, caregivers and support workers are welcome.
Meet once a month with people who share your interest and concerns about diabetes. Share tips and information, learn about community services and how to live well with diabetes. All groups are led by a registered dietitian. The topics and activities are decided by each group. Families and caregivers are welcome.
A registered dietitian and diabetes nurse educator help you learn how to feel better and avoid unnecessary complications from diabetes. Family and caregivers are welcome. Childcare and TTC tickets are available on advanced request. Classes include: Level 1: Introduction to Diabetes Management 1 session (4 hours)
1 ½ hour tour of a grocery store with a registered dietitian. Take an eye-opening tour of a supermarket with a dietitian, and learn about the best food choices for your health and your food dollar. Groups tours (5-12 people) only.
Adults meet twice a month from April to October to enjoy rooftop gardening. Each session includes time to work in raised beds, as well as presentations by a guest speakers and/or gardening expert. It is a great way to get some fresh air, do some gardening, learn new things, and have some fun! Help us plant, water, weed, and harvest from our raised garden beds.
An English-speaking dietitian and diabetes nurse educator lead classes with simultaneous translation in Tamil. Family and caregivers are welcome. 1 session (4 hours)
An Urdu/Hindi-speaking dietitian leads classes at the East End Community Health Centre. Family and caregivers are welcome. 4 sessions (2 ½ hours each)
