Information is the first step to good health. Learn more about what you can do to stay healthy -- and feel your best! A team of health promoters, social workers, dietitians, doctors and nurse practitioners provide a wide range of health promotion and education programs.

Available Programs:

The South Riverdale Community Health Centre offers support and information for early detection and living with cancer.
 Information, programs and services to help you manage your diabetes, feel better and avoid complications. All programs and services are free, and open to caregivers and family. Some diabetes services and programs are offered in Chinese and English.
The East Toronto Hepatitis C Program is a partnership between South Riverdale Community Health Centre, Regent Park Community Health Centre and Sherbourne Health Centre.
A specialized healthcare provider works with the health services and urban health teams to provide one-to-one care and education on asthma. The Primary Care Asthma Program (PCAP) is part of Ontario’s Asthma Plan of Action.
