Chronic Disease Prevention and Management

Interdisciplinary team focused on chronic disease prevention, management and education.  Includes, but not limited to asthma, COPD and diabetes.

Doctors, nurses, nurse practitioners, social workers, outreach workers, respiratory therapists, dietitians and chiropodists offer primary health care, counseling, education and social services to residents of our catchment area. Priority is given to people who face barriers to services such as low literacy or income; homelessness, substance use and/or mental health issues; gender and sexual orientation; language, race and culture.


The South Riverdale Community Health Centre offers support and information for early detection and living with cancer.

The Toronto Community Hepatitis C Program (TCHCP) is a partnership between South Riverdale Community Health Centre, Regent Park Community Health Centre and Sherbourne Health Centre.
