PRIMARY HEALTH CARE NURSE PRACTITIONER

PART-TIME POSITION (0.8 FTE)

The South Riverdale Community Health Centre requires a RN(EC) who will function as an effective member of our Newcomers and Families Team. The Centre provides programs and services predominantly aimed at people who live in South East Toronto including people of Chinese heritage, people with substance use and/or mental health challenges, seniors, new immigrants / refugees and young parents. The CHC is core funded by the Ministry of Health and Long Term Care.

Responsibilities: 
  • Conducts comprehensive health assessments of the well person throughout the lifespan.
  • Assesses and manages episodic and stable chronic conditions commonly seen in primary health care.
  • Provides supportive and lifestyle counselling which may include preventative counselling, brief supportive counselling or short term crisis management when indicated.
  • Writes prescriptions and orders laboratory tests within the RN(EC) scope of practice.
  • Provides pre- and post-natal care, which includes collaboration with CHC physicians, "maternity care" physicians, midwives or health care providers chosen by the client, as needed.
  • Makes home visits to clients who live within the catchment area as necessary.
  • Makes internal referrals to other SRCHC staff and programs as appropriate.
  • Facilitates referral of clients to specialist, hospital or community resources as appropriate.
  • Participates in case conferences with other providers to ensure coordination of services to the client. Works as a member of an interdisciplinary team.
  • Ensures that standards of client care are maintained according to accepted College of Nurses standards, the Regulated Health Professions Act, and the policies of SRCHC.
  • Performs necessary back-up clinical functions for other nursing staff in their absence.
  • Provides primary care services in an offsite location, within an adult school setting on a weekly basis.
  • Assists and works with community groups in the development of community outreach programs as directed.
  • Participates in the development of, and gives support to, policies/strategies which are in the interest of public health and which support the recognition of the determinants of health.
Qualifications: 
  • Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario.
  • RNAO membership.
  • Baccalaureate degree in nursing from a recognized university.
  • Three to five years nursing experience in a community setting, hospital or public health setting and experience in primary care.
  • Demonstrated expertise in medical laboratory procedures, thorough knowledge and understanding of infection control and preventative practices.
  • Demonstrated ability to work effectively in a multi-disciplinary environment.
  • Demonstrated commitment to and knowledge of community- based health care.
  • Awareness of and sensitivity to the health issues of a diverse community and experience working with disenfranchised populations.
  • Non-judgmental and non-confrontational attitude.
  • Excellent communication, interpersonal, organizational and administrative skills that include computer literacy.
  • Ability to work flexible hours.
  • Fluency in a language relevant to the Riverdale Community in addition to English is considered an asset.
