Registered Nurse

The South Riverdale Community Health Centre (“SRCHC”) is a non-profit, multi-service center that provides primary health care, social and community outreach services with an emphasis on health promotion to residents of Riverdale and South East Toronto.

 

In partnership with WoodGreen Community Services, this South Riverdale Registered Nurse position will provide primary health care and referral for the male tenants of the First Step to Home Housing Program of WoodGreen Community Services, all of whom are older adults living with concurrent substance use and mental health issues.

 

Working as part of a team including a SRCHC NP, Social Work/Case Management and PSW support from WoodGreen Community Services and Community Outreach Program in Addictions (COPA), the successful candidate will have experience and comfort working with marginalized homeless populations and will use a harm reduction approach working towards promoting wellness with frail, at risk seniors who have long experiences of homelessness, poverty, substance use and challenging physical and mental health issues.

 This contract position will run to March 2014, with the possibility of renewal.  

Responsibilities: 

 

  • Assesses and manages episodic and stable chronic conditions commonly seen in primary health care
  • Visits clients as necessary
  • Within scope of practice of RN conduct health assessment, including history and partial physical exam for clients
  • Within scope of practice, communicate with clients and external providers and services regarding the tracking of test results, follow-up appointments, written prescriptions and triage urgent care
  • Makes internal referrals to other SRCHC staff as appropriate
  • Facilitates referral of clients to specialist, hospital or community resources as appropriate
  • Participates in case conferences with other providers to ensure coordination of services to client
  • Works as a member of an inter-professional team providing comprehensive primary health care, counseling & support and coordination of services to marginalized, substance using, frail older adults who may also be living with mental health issues
Qualifications: 

 

  • Current registration as a Registered Nurse with the College of Nurses of Ontario & RNAO membership
  • Baccalaureate degree in nursing from a recognized university
  • Five years nursing experience in a community based, hospital or public health setting and experience in primary care
  • Strong assessment, case management and clinical skills
  • Demonstrated experience in providing care to older adults with mental health and substance use issues, from a recovery and harm reduction approach
  • Demonstrated ability to work independently with minimal supervision
  • Demonstrated commitment to and knowledge of community-based health care
  • Proven ability to navigate health care system and minimize barriers through service delivery for low income older adults with mental health and substance use issues and experiences of homelessness
  • Demonstrated ability to work effectively in an inter-professional team
  • Ability to work flexible hours
