ADMINISTRATION - PROGRAM, TORONTO CENTRAL DIABETES SELF-MANAGEMENT PROGRAM

FULL-TIME POSITION (35 HOURS/WEEK)

The South Riverdale Community Health Centre (SRCHC) is a non-profit, multiservice centre that provides primary health care, social and community outreach services with an emphasis on health promotion to residents of Riverdale and South East Toronto.  The Centre is the host organization for the Toronto Central Diabetes Self-Management Program, funded by the Ontario Diabetes Strategy of the Ministry of Health and Long Term Care.  The goals of the DSMP are: to provide self-management education to individuals living with diabetes and their care-givers; and to enhance the capacity of health care providers to assist individuals who are self-managing diabetes.

The Administration – Program position works in collaboration with the DSMP Program Coordinator in facilitating client flow through the community based diabetes self-management education programs. This may involve booking clients, booking space, arranging for catering, file management, data entry, program material and supply preparation. This position is responsible for the maintenance of related databases.  This position requires providing service to a range of clients, including marginalized populations.

Responsibilities: 
  • Booking program participants, managing incoming referrals, making reminder phone calls
  • Coordinates the availability of supplies, program materials and educational literature across multiple sites
  • Maintains program database
  • Coordinates activity of volunteers in consultation with the coordinator
  • Collaborates with program team in the production of program material, brochures, flyers
  • Coordinates mass mailings
Qualifications: 
  • Minimum College Diploma achieved
  • Clerical experience preferred
  • Ability to multitask and simultaneous attention to detail is essential
  • Awareness of and sensitivity to the health issues of a diverse community and marginalized populations
  • Demonstrated ability to work independently and as part of a team in a fast paced environment
  • Excellent communication skills
  • Advanced knowledge of Microsoft Office (Word, Power Point, Access, Excel)
  • Experience with data entry
  • Ability to be flexible
