Health and Strength Action Group

The Health and Strength Action Group is a group of local community members working with South Riverdale Community Health Centre on issues of poverty and health.

Advocacy & Assistance

Individuals often need more than one service, as well as support and follow-up. An experienced case coordinator can help you find the community services you need.

Common issues include:

  • food
  • housing
  • clothing
  • education (schools)
  • legal rights
  • government forms


Rooftop Gardening & Wellness Program

Since 2000, SRCHC has run a rooftop garden group in the spring-fall months. In 2011/12, with the help of a grant from the Government of Ontario Healthy Communities Fund, we were able to expand the program to provide more staff support, workshops and field trips at SRCHC, Central Toronto Community Health Centre and Flemingdon Health Centre.

Diabetes Support Group & Community Kitchen (Chinese)

Informal discussions and healthy meals prepared in our community kitchen with a Chinese-speaking dietitian, nurse, and other people living with type 2 diabetes or pre-diabetes. Caregivers are welcome. Registration required. 

Living Well with Diabetes Classes (English)

Group counselling, education and support for individuals with type 2 diabetes and pre-diabetes, for those indviduals who can benefit from a group learning environment. Groups are facilitated by registered diabetes educators (RN and RD) as well as a registered social worker.

Counseling & Case Coordination

Harm reduction counselors offer support and information on safer drug use, health and nutrition, relationships and personal safety, detox and methadone treatment options. As well as addressing drug, health and legal issues, outreach workers and counselors help clients secure basic needs such as housing and food.
