CHC Food Security Network

The network is a coordinating body that provides leadership to the issue of food security in CHC's.

The network communicates food security as encompassing:

  • food and health (food related to illness)
  • food and access (poverty, culturally approppriate)
  • food as sustainability (environment, agriculture)

Case Management

Community support workers help high-risk clients find and use the services they need. Case management is an intensive service offered to clients who are homeless or coping with settlement issues, poverty, addiction and/or mental health problems.

Advocacy & Assistance

Individuals often need more than one service, as well as support and follow-up. An experienced case coordinator can help you find the community services you need.

Common issues include:

  • food
  • housing
  • clothing
  • education (schools)
  • legal rights
  • government forms

 

Plain Language Group

The Plain Language Group is a group of volunteers who review printed health information (pamphlets, posters, booklets) to make sure it is easy to read and understand. We do three things: 1. We advocate for health literacy in our health centre and our community. 2. We learn about plain language and improve our own skills. 3. We work with people who want to write in plain language.
