Primary Care Asthma Program (PCAP)

A specialized healthcare provider works with the health services and urban health teams to provide one-to-one care and education on asthma.

The Primary Care Asthma Program (PCAP) is part of Ontario’s Asthma Plan of Action.

Services include:

  • Education - sharing information, answering questions and distributing educational materials
  • Monitoring - breathing tests (spirometry), peak flow monitoring and respiratory assessments
  • Self-Management Training - puffers (respiratory medication), self-monitoring and creation of an asthma action plan
  • Home Visits - doctors and nurse practitioners visit shelters and homes upon request
  • Presentations and materials for schools and community groups (upon request)
