Counseling & Case Coordination

Harm reduction counselors offer support and information on safer drug use, health and nutrition, relationships and personal safety, detox and methadone treatment options.

As well as addressing drug, health and legal issues, outreach workers and counselors help clients secure basic needs such as housing and food.

After connecting people with the appropriate resources and services, including treatment agencies, workers often maintain a relationship, following up and coordinating their client’s progress.

This program is a part of the COUNTERfit.
COUNTERfit is a harm reduction program that began in response to concerns that users of illicit drugs were putting the community at risk.
COUNTERfit offers a needle exchange program, safer crack stem kits and other harm reduction materials and supplies to illicit drug users. The program also offers counseling, information and referrals and case coordination, as well as access to the broader health system.

Eligibility:

People who have difficulty accessing conventional health and social services because of drug use, homelessness, and/or mental health problems.

Service Boundaries: Don Valley Parkway to Victoria Park Avenue; Eglinton Avenue to Lake Ontario

Hours:

Monday to Friday, 10 a.m. to 5 p.m. (COUNTERfit offices)

Monday & Wednesday, 1:30 p.m. to 5 p.m. (Housing worker from Woodgreen Community Services)
