Case Management

Community support workers help high-risk clients find and use the services they need.

Case management is an intensive service offered to clients who are homeless or coping with settlement issues, poverty, addiction and/or mental health problems.

Common issues include:

  • accessing medical services without health insurance
  • applying for benefits/support without a fixed address
  • finding and keeping housing
  • finding work/employment
  • legal issues
  • depression

Eligibility:

Live within service boundaries.

Languages:

  • English
  • Mandarin
  • Cantonese

Hours:

Vary (please enquire).

Required.
